A 34-year-old man has dead and a child was taken to hospital with life-threatening injuries after a three-vehicle collision in Lombardy, Ont., on Friday. 

Leeds County OPP responded to a head-on collision involving two cars and an ambulance at around 5 p.m. 

Police say one driver was pronounced dead at the scene, and a child from that car was airlifted to a trauma centre.

Police say the occupants of the ambulance suffered serious injuries while the driver of the other vehicle sustained minor injuries. 

The ambulance had no patients on board. 

The cause of the crash is still under investigation, and Leeds County OPP are asking anyone with dashcam footage to contact them or Crime Stoppers.
